Congenital Erythropoietic Porphyria and Protoporphyria
Core Pathogenesis & Mechanism:
Fundamental Cause:
Rare hereditary enzyme deficiencies in the
heme biosynthesis pathway
.
General Mechanism:
Failure to process porphyrins leads to their accumulation in erythrocytes, skin, bone, and other tissues.
Phototoxicity:
Deposited porphyrins are photocatalytic; sunlight exposure creates oxygen radicals that damage tissues and vessels.
Congenital Erythropoietic Porphyria (CEP):
Specific Deficiency:
Decrease in
uroporphyrinogen III synthase (URO-synthase)
activity.
Clinical Presentation:
Erythrodontia:
Reddish-brown discoloration of teeth and bones that
fluoresce pink
under ultraviolet light.
Photosensitization:
Hyperemia, vesicles, and necrosis on unpigmented or thin skin.
Anemia:
Fragile red blood cells lead to chronic hemolytic anemia and
splenomegaly
.
Urine:
Amber or reddish-brown due to excess porphyrin excretion.
Affected Species:
Cattle:
Primarily
Holstein
and South African Shorthorn breeds.
Cats:
Feline models show similar mutations to human CEP.
Swine:
Inherited as an autosomal dominant trait, featuring discolored teeth but
no anemia or photosensitivity
.
Genetics:
Usually inherited as an
autosomal recessive
trait (except in swine and certain feline variants).
Bovine Congenital Erythropoietic Protoporphyria (BCEPP):
Specific Deficiency:
Deficiency in
ferrochelatase
activity.
Distinctive Characteristics:
Neurological Signs:
Unique to the bovine form; includes
intermittent seizures and ataxia
triggered by sunlight.
Absence of Typical CEP Signs:
Does
not
manifest with pink teeth or anemia.
Skin Lesions:
Erosion and moist exudative dermatitis, often in the gluteal area.
Affected Breeds:
Primarily
Limousin
and Blonde d’Aquitaine cattle.
Genetics:
Inherited in an
autosomal recessive
pattern.
Protoporphyria in Other Species
Dogs:
Described in a
Clumber Spaniel
with small stature and
protoporphyric hepatopathy
(liver involvement).
Treatment (Dog Case):
Involved antioxidants, biliary support (ursodeoxycholic acid), and strict sunlight avoidance.
